Invasive coronary angiography has long been considered as the gold standard method for diagnosis of obstructive coronary artery disease. However, there is frequent discordance between the visual assessment of intermediate stenoses and their consequence on myocardial perfusion. The benefit of revascularization by percutaneous coronary intervention (PCI) over optimal medical treatment has been repeatedly questioned in patients with stable coronary artery disease possibly related to the reality of coronary stenoses and/or myocardial ischemia. Invasive functional testing using fractional flow reserve (FFR) has emerged as a discriminant tool to guide the strategies of PCI.
